We have what is called an Open List which means we are happy to accept new patients.
You must live within our catchment area of Bishop Auckland, Shildon, West Auckland, Coundon, Leeholme.
You need to come into reception and fill in a couple of short forms.
You also need some form of ID such as passport or photocard driving licence plus a bill that shows your home address such as council tax, water bill, landline phone bill.
If you are aged over 16 you will be invited to have a 15 minute appointment with one of our nursing assistants to run through your form plus you will have the opportunity to have your blood pressure taken.
